There is room for up to 20 potential prospects, but it is unlikely that any CSR will list that many in a given month. The document is a Word file containing an easily editable table. Provide copies of the .doc file to all personal lines reps in your office that you want to use it. They may complete it on their computers (using the tab or arrow keys to move around) or go retro and fill it in by hand. Each column on the form is self-explanatory.
